At 10:12 a.m., the second patient of the morning still has not arrived. The front desk can log the missed visit, offer the slot to the waitlist, and send a neutral rebooking message. It should not decide from memory whether this patient gets a fee, a warning, or an exception.
Policy controls at a glance
- Define the event before assigning a consequence.
- Keep reminders useful and privacy-limited.
- Put hardship, access, and clinical exceptions in the normal workflow.
- Measure the policy by appointments offered, missed, recovered, waived, and appealed.

MEDICAL OFFICE APPOINTMENT AND NO-SHOW POLICY
Purpose
We use appointment time carefully and want patients to have a clear route to
cancel, reschedule, explain an exception, and return to care.
Definitions
No-show and grace period: _________________________
Late-cancellation window: _________________________
Late-arrival review: ______________________________
How to cancel or reschedule
Phone and hours: __________________________________
Portal or other approved route: ___________________
After-hours instructions: _________________________
Reminders
We may send appointment reminders through __________. Reminders are a
courtesy; patients should keep their own appointment information current.
First event
Neutral outreach and rebooking steps: _____________
Repeated events
Progressive review, owner, and approved consequences: __________
Fees, if approved
Amount or formula after required review: __________
Charging, waiver, disclosure, and appeal rules: ___
Exceptions and review
Tell us if an emergency, hospitalization, transport failure, disability-related
need, communication barrier, or other circumstance affected the appointment.
The designated reviewer is __________ and uses the same documented criteria.
Clinical continuity
A missed visit does not authorize staff to ignore an urgent message, pending
result, active treatment issue, or other clinical concern. Route those items to
the named clinical role and response path: ______________________.
Questions or appeal
Contact __________ by __________. We document the review and response.
Effective date: __________ Owner: __________ Review date: __________
This template deliberately leaves fees and time windows blank. The practice has to earn those lines through local review.
Define the event in plain language
Decide how late is late, when a cancellation becomes late, and what happens when the clinic itself is running behind. Include telehealth connection failures, transport problems, and appointments the patient reasonably believed were cancelled.
A useful event log needs facts:
- appointment start time;
- arrival, call, cancellation, or connection time;
- reminder channel and delivery status;
- outreach attempt;
- rebooked, waitlist-filled, or unresolved outcome;
- exception requested and reviewer;
- consequence, waiver, or appeal result.
Avoid labels such as “habitual” or “noncompliant” in the scheduling log. Count events and record the reviewed outcome.
Reminders should disclose less, not more
HHS OCR says the HIPAA Privacy Rule permits appointment reminders and other communications about care. The same guidance advises limiting what is left on an answering machine to information reasonably needed for the communication, such as the provider’s name and callback number.
Let patients state communication preferences and keep contact details current. Test voicemail, text, email, and portal wording with synthetic accounts. A reminder should not reveal a specialty, diagnosis, procedure, or sensitive clinic name when a simpler confirmation will do.
For the wider scheduling-system decision, see the medical office scheduling software guide. For automated recovery messages, the patient follow-up automation guide keeps clinical replies with a human.
Use an exception lane every time
Do not bolt on an exception after somebody complains. Put it in the standard event screen.
The staff member logs the request. A named reviewer checks the policy criteria. A clinical concern goes to the clinical lane. A disability or discrimination concern goes to the approved compliance lane. The patient receives the outcome and an appeal route.
The same workflow can still produce different outcomes when facts differ. Consistency means using the same criteria and review, not pretending an emergency and a forgotten appointment are identical.
Escalation is not automatic dismissal
Progressive steps can include a reminder, a copy of the policy, a scheduling conversation, or a manager review. If the practice considers ending the relationship, stop the scheduling automation and open a separate case.
The AMA ethics opinion on terminating a patient-physician relationship requires notice long enough to permit the patient to secure another physician and appropriate help transferring care. The exact legal and operational path varies. Use the state-aware patient dismissal template only after that review.
Pending results, active medication questions, postoperative care, or other urgent issues need owners regardless of appointment status. A no-show flag cannot close a clinical task.
One monthly table tells you whether the policy works
| Measure | Count | What to inspect |
|---|---|---|
| Appointments offered | Denominator and clinic mix | |
| Late cancellations | Timing and reminder delivery | |
| No-shows | Repeated patterns and access barriers | |
| Slots filled from waitlist | Recovery, not punishment | |
| Exceptions requested | Whether patients can find the route | |
| Exceptions approved | Consistency by reviewer and reason | |
| Fees or other consequences applied | Contract and policy compliance | |
| Appeals changed | Policy ambiguity or staff error |
Compare the same definitions month to month. Do not claim the policy reduced no-shows when the office changed the denominator, grace period, or appointment mix.
We think the recovered-slot row is the useful one. A stern policy that leaves the calendar empty has solved the wrong problem.
Product boundary
Patient Square is an AI clinical platform. Practice Copilot brings the whole practice under one AI copilot: an ambient AI Medical Scribe that hands back a structured SOAP note, ICD-10 suggestions, and a prescription draft minutes after the visit, plus a bundled AI EHR, scheduling, and messaging as you move up the plan. Hospitals get Hospital Copilot.
That scope does not promise automated fee assessment, contract interpretation, patient dismissal, or a legally approved no-show policy. Keep those decisions with the practice.
If scheduling and documentation fail at the same handoff, review the current Practice Copilot scope. Verify the current US plan scope and security controls before adopting any workflow.
